Autism and ADHD referrals:

If you are asking us to consider autism and/or ADHD assessment, the family and school will need to complete an additional information document together. Please follow the instructions on this page. We will not be able to proceed with the request for help unless you have uploaded the additional information document. (Please note, ADHD referrals are accepted only for children aged 6 years and above).
